I, ________________________________, hereby certify that my dog(s) is (are) in good health and has (have) not been ill with any communicable condition in the last 30 days.  

I hereby certify that my dog(s) has (have) not harmed or shown aggressive or threatening behavior towards any person or any other dog.  

I understand and agree that in admitting my dog(s) to Flying High Pet Resort all staff has relied on the above representation regarding the health and behavior of my dog(s).  I further understand and agree that I am solely responsible for any harm caused by my dog(s) to another dog or staff member while my dog(s) is/are attending Flying High Pet Resort.  I agree to release Flying High Pet Resort staff of any liability for any problems which may develop, provided reasonable care and precautions are followed, and I hereby release them of any liability of any kind whatsoever arising from my dog(s) attendance and participation at the spa.  

I understand and agree that any problems which develop with my dog(s) will be treated as deemed best by staff of Flying High Pet Resort at their sole discretion, and that I assume full financial responsibility for any and all expenses involved.  

I further understand that Flying High Pet Resort reserves the right to refuse admittance to any dog for any reason, and that no aggressive dogs will be allowed to attend the spa. Flying High Pet Resort reserves the right to refuse admittance to pit bulls and pit bull mixed breeds.  

I certify that I have read, understand, and agree to the policies, conditions, and statements of Flying High Pet Resort as set forth in this Owner Agreement, including the following:  

DAYS AND HOURS:  Daycare is open Monday through Friday, 7:00 am to 6:30 pm. Daycare is available from 7:00 am to 4:00 pm on Weekends and Holidays. Drop off is between 7:00am and 10:30am, no reservation needed.  Boarding is available year-round, with pick up and drop off by appointment from 7:00am to 2:00pm weed days and 7:00am to 12:00pm weekends and holidays. Any dog that has not been picked up from daycare by 6:30 OR 4:00 pm will be charged a $15 late fee.  Any dog remaining after 6:30 OR 4:00 pm will be boarded with an additional overnight fee of $70+.  

RESERVATIONS: Boarding Reservations are required.  Cancellations must be made no later than 24 hours before the reserved day in order for a credit to be applied.  If there is no notice of cancellation, a full night’s fee of $70+ will be charged.
